3. Submit the Application: Hand in the completed and signed application form along with the required documents to the Assistant Director or Deputy Director at the regional office.
4. Obtain Acknowledgment: After submission, request a receipt or acknowledgment from the Assistant Director or Deputy Director. Ensure that the receipt includes vital details such as the submission date, time, and a unique identification number if applicable.
