Application Procedure

Interested individuals can apply for the scheme through an offline process. Below are the steps to follow:

  • Step 1: Visit the District Differently Abled Welfare Office during office hours to request a hard copy of the application form from designated staff members responsible for handling applications.
  • Step 2: Complete the application form by filling in all required fields. Ensure that you attach photocopies of all necessary documents (self-attestation may be needed).
  • Step 3: Submit the filled and signed application form along with the attached documents to the District Differently Abled Welfare Officer.
  • Step 4: Obtain a receipt or acknowledgment from the District Differently Abled Welfare Officer upon submission. This receipt should clearly indicate the date and time of submission, along with a unique identification number, if one is provided.

  • Certificate of Intellectual Disability, indicating the percentage of disability.
  • Identity proof (Aadhaar card, voter ID, etc.)
  • Address proof (utility bill, rental agreement, etc.)
  • Passport-sized photographs (if required).

Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).
Step 3: Submit the duly filled and signed application form along with the documents to the District Differently Abled Welfare Officer.
Step 4: Request a receipt or acknowledgment from the District Differently Abled Welfare Officer to whom the application has been submitted. Ensure that the receipt contains essential details such as the date and time of submission, and a unique identification number (if applicable).
